Referrals
- call the Intake Dept. at 716.285.3425. Referrals can be initiated by
self, family, treatment team, clergy, or any community agency.
Referrals can also be made through the
Niagara County Department of Mental Health Single Point of
Access at 716-439-7412
Applications
- requires a physicians prescription for restorative services
(psychiatric rehabilitation), SPMI diagnosis, a physical, social
history, and financial information.
Tour
and interview with Intake staff.
Application
review by Intake Committee.

ADMISSION CRITERIA
Functional Criteria
at least 18 years of age
primary diagnosis of a Serious and Persistent Mental Illness (SPMI)
self preserving in a fire emergency
understanding and willingness to work with physicians and staff on a
therapeutic medication regime
ambulatory (call for availability of one wheelchair bed), continent,
provides adequate self care when stable
free
of acute debilitating physical disorder

Program Criteria
applicant desires growth toward least
supportive living environment
applicant agrees to follow residence guidelines and participate in the
rehabilitation process
applicant desires maintenance of clean and
sober
status and addresses relapse issues

agrees to pay a monthly fee based on entitlements and ability to pay
