Rapid Re-Housing Case Manager (YHDP)

Division: REACH (Youth Services)

Program: Transitional Housing/Rapid Re-Housing Program (TH-RRH)

General Description: Assumes responsibility for the operation of the Rapid Re-Housing and Case Management component of the YHDP program, including referrals, admission assessments and Intake. Maintains Housing Lists which meet acceptable rental and safety environments for young adults and families. Ensures living skills progression for young adult transition to permanent housing options. Completes Individual Service Planning with all Rapid Re-Housing young adults. Monitors program operations and budget. Assures required records and communications are completed and maintained. Utilizes Peer Support Specialists, reporting any concerns to Transitional Supervisor. Works in close and cooperative communications with Transitional Supervisor and additional Rapid Rehousing Case Managers to ensure integration and compliance with all aspects of YHDP Performance Benchmarks and Program Standards. Follows written procedures of regulatory and certifying bodies (HAWNY, HUD, Medicaid, DSS, OSHA, Health Department, etc.) as well as upholding guidelines, procedures and policies of Community Missions, Inc. On-Call responsibilities in rotation with the Team.

Preferred Qualifications: Bachelor’s Degree in Human Services or related, Minimum 21 years of age; Red Cross Training; Infraction-clear NYS Driver’s License. Previous experience with transition age youth, homeless youth or adults; experience in housing identification and standards, client assessment and linkage, case management and service coordination, landlord/tenant negotiations, family and individual counseling skills, Small group leadership skills; ILS/DLS skills work; Conflict resolution skills; Strong supervisory skills and experience leading teams.

Salary Range: $35,320 - $48,00 annually (based on experience)

Hours: Full-Time (some evenings required for Service Plan meetings or specialized needs.)
