General Overview: Provide person-centered case management services to individuals receiving mental health housing services, including service planning, coordination of community resources, and advocacy. Develop and monitor Individual Service Plans, maintain Medicaid documentation, and ensure accurate recordkeeping and billing. Coordinate admissions, discharges, benefit procurement, and healthcare access while supporting residents in achieving their goals. Participate in an on-call rotation and help ensure quality services
Qualifications: Preferred Bachelor’s Degree in Social Work, Human Services, Community Mental Health or related field. Preferably one year of Human Services. Must have valid NYS Drivers License. (Due to upcoming renovations, this position will be based in Niagara Falls for approximately 9 months before returning to the North Tonawanda location.)
Hours: Full Time
Salary Range: $42,000 - $51,500, annually.
