General Description: Oversight and management of the individual cases of all Supportive Housing tenants. Development of Tenant Individual Support Plans. Tenant linkage to Community Service Providers. Observation, documentation and reporting of tenant status.
Accountable To: Supportive Housing Manager
Preferred Qualifications: Bachelor's degree in Social Work or related field preferred. One year experience in Human Services or related field. Valid NYS Driver's License.
Salary Range: $38,355 - $42,764 annually
Hours: Full Time
