Juvenile Supported Case Manager

Program: Juvenile Supported Case Management (Youth Services)

General Overview or Main Functions: Assumes responsibilities for the direct provision of Supported Case Management (SCM) programming for (8) eight juvenile probationers and foster care re-entry youth and families. Ensures case management components comply with a restorative justice philosophy and youth violence reduction strategy. Keeps accurate statistical and case records. Works in conjunction with County and City/Town Police Departments and Niagara County Department of Social Services and/or Probation officer assigned to the case in providing updates regarding the youth’s progress, as well as discussing any problems that may arise. Meets with youth and their family or legal guardian three (3) times per week (2 face-to-face sessions, and one virtual) to discuss progress on service planning as well as provide counseling and other related support. Assures monitoring and effectiveness of Mental Health Consultants and works in cooperation with Family Support Providers and additional program case managers. Carries an agency cell phone for 24-hour on-call contact ability.

Preferred Qualifications: Bachelor’s Degree in Social Work, Social Science or related field (Master’s preferred); 21 years of age or older; clean NYS Driver’s License; Must have own reliable transportation; NYS State Central Registry Clearance, NYSOMH Fingerprint Clearance. Minimum two years experience working with at-risk youth; Knowledge of Juvenile Justice System and Family Court process; Crisis Intervention skills; Conflict-resolution skills; Small group leadership skills; Excellent Record-keeping and Writing skills, Computer Skills, Family and Youth Engagement skills.
