General Overview: The Rapid Re-Housing Case Manager helps individuals and families move from homelessness to permanent housing with a focus on long-term stability. Responsibilities include assessments, creating housing plans, assisting with searches, completing HUD inspections, and coordinating move-ins. The role provides ongoing case management in budgeting, income, employment, and self-sufficiency, while managing rental and utility assistance. Accurate documentation in HMIS and participation in team meetings and community coordination are key to supporting client success.
Qualifications: Bachelor's Degree in a related field preferred and at least one year of experience in human services. Strong organizational, communication, and time management skills. Proficiency in Microsoft Office and internet research is essential. A valid NYS drivers license with a clean driving record is required.
Salary Range: $40,000-$45,000, annually.
